תשע עשרה קבלות המעידות שזוג נעליים נמסר לתושב יהודי בירושלים (חתומות בחתימתו של מקבל הנעליים). הונפקו מטעם "ועד הסיוע המאוחד האמריקני לנגועי המלחמה היהודים", ירושלים, תרפ"א 1921.
בכל קבלה נוסח הצהרה מודפס: "קבלתי מקרן יעקב נתן לונדון, זוג נעליים שתמורתן עולה סך מאה עשרים וחמשה גרו"מ [גרוש מצרי]", ובשוליו חתימתו של המקבל. בשוליים העליונים מופיעה חותמת הוועד ובשוליים התחתונים חותמת נוספת לאישור ("נבחן").
19 קבלות. 21X13 ס"מ בקירוב (שוליים חתוכים בקווים לא-ישרים). מצב כללי טוב. כתמים, קמטים וקרעים קלים בשוליים (קצתם חסרים).
