Fragebogen zur erstmaligen Meldung der Heilerufe - A detailed questionnaire to fill in personal details of the Jewish physician Edward Falik from Lemberg (Lvov) who was recruited by the Nazis as a 'doctor on duty', filled out in Falik's handwriting, and his passport photo. And a large stamp in the shape of a Star of David to indicate its origin. Falik did not survive the war. April 1, 1942. German.
Among other things, the questionnaire covers his medical training and his distant past, such as questions related to his military past during World War I, whether he participated in the Polish army, and if so to what degree, and whether he participated in any organizations, other specializations he studied, other jobs he worked for, What he did in 1940 and 1941 and more.
