The translation of the ballot is: " Do you recognize our leader Adolf Hitler, the Liberator of the Sudetenland, and give your vote to the candidate of the National Socialist German Workers' Party headed by: 1. Adolf Hitler, 2. Konrad Henlein, 3. Karl Hermann Frank?"
