An application form for financial support from the Relief Committee for Refugee Rabbis (From Russia and other countries). The application, which includes valuable personal information, was filled out by the Admor Rabbi Ashlag and he signed his name, "Yehuda Leib, son of our teacher Rabbi S.I. Ashlag." Dated and signed 29 Elul 1940 (erev Rosh Hashanah!).
