דף מודפס על גבי נייר עבה, בפורמט רחב ובאותיות גדולות ומנוקדות, עם נוסח הברכה שאומר אבי-הבן לאחר שהמוהל מל את בנו.
בראש הדף נדפסה ההוראה ביידיש: "לאחר שהמוהל גומר את החיתוך, אומר אבי הבן את הברכה הבאה", ואחריה מופיע נוסח הברכה, עם שיבוש במילה "להכניסו", אשר נדפסה כאן "להכניתו" (בהגייה האשכנזית, האות תי"ו שאינה דגושה, נהגית כסמ"ך).
