Attribued to Rabbi Chaim Vital. The press of Avraham ben Yitzchak Isaac and Eliyahu ben Yaakov Segal.
This book had several variants matching their designated readership: a variant for Misnagdim with an approbation by Rabbi Chaim of Volozhin and Rabbi Mordechai of Koretz and a variant for Chassidim in which the above approbations were replaced with those by Rabbi Levi Yitzchak of Berditchev and Rabbi Yehuda Leib of Voltshisk.
There was also a Chassidic edition with typographic variations. See: H. Lieberman, Sinai, 70, pp. 171-176 and Ohel rachel section I, pp. 127-131.
