קמיע לשמירה מפני פגע רע ומחלה, עם שמות מלאכים והשבעות, סמל מגן-דוד, חותמות מלאכים וטבלאות צירופי שמות קדושים. "ותצילה מכל מיני חולי ומחלה, וכל סיבה ומקרה ופגע רע ועין הרע, ומכל כאב ומכאוב... שתפקוד ותזכור לטובה לנושאת קמיע זה...".
