Nazi registration form for a Jewish doctor in Galicia (July 1942)

Fragebogen zur erstmaligen Meldung der Heilberufe – “Questionnaire for Initial Registration of Medical Professionals” - Official registration form for a Jewish medical professional named Moshe Grinz, issued by the administrative authority of Distrikt Galizien (Eastern Galicia) following its annexation into the General Government under Nazi occupation. Dated March 30, 1942 – at the height of the deportations and extermination period. The document bears Nazi stamps as well as a Star of David stamp.


The form includes Moshe’s personal details and passport photograph, as well as handwritten entries. Moshe was born on May 16, 1894, in Rawa Ruska (then part of Galicia). His registered address: Rawa Ruska, Lwowska 11. Marital status: Married; wife’s name: Gitel, born in 1898. The couple had three children, born in 1923, 1926, and 1928.

Under the "Religion" field, it is recorded: “Jewish.” In the professional section, his occupation is listed as Desinfektor (sanitation/disinfection worker). His place of employment is recorded as Bürgermeisteramt Rawa Ruska – the Mayor’s Office of Rawa Ruska – an official municipal position. The form also includes the names of his parents, paternal and maternal grandparents, and his mother’s maiden name, along with many additional questions answered “yes” or “no.”

Questions include: – Did you have a license to practice a medical profession in Soviet-controlled territory? Did you serve in the Red Army up to July 1, 1941? The form is printed in four languages: German, Polish, Russian, and Ukrainian, and bears a Red Star of David as well as a swastika stamp. Dated July 6, 1942, this was a mandatory document the Nazis required Jewish doctors, pharmacists, nurses, midwives, and medical professionals to submit in order to register with the occupation authorities and enable systematic, bureaucratic oversight. The form is stamped: Der Kreishauptmann im Distrikt Galizien – District Commissioner representing the German administration in the Galicia region.
